Why use a form at all?A fast and bold parallel could be... the pay office never let you use a blank page for reporting your work hours. I have seen many examples where people just send an e-mail asking for detailed information, or attach a spreadsheet to be filled out. A problem always arises when the replies returns, and one person has changed the layout of the document, or another has forgotten to answer some vital questions. With QR Forms, you can prevent these kinds of mistakes completely. Once you have made it, a user cannot change the design of a QR form. The form can be set up to ensure that a user replies to certain questions before the form can be sent back to you. There are some more advanced methods to gather replies Learn more

What you need to get started. You (the designer) need the QR Designer program. Download

My first form.
First, start the designer. Since I am a function-oriented person, I will place the text boxes first. To do that, click on the ‘ab’ button. Now you will see a textbox on the form. Drag it down to a nice place. Repeat this two times. To inform the end user of the information you want in the boxes, place a label (the one with an ‘A ’on it) just above the text boxes and double-click on the new label. In the property editor that pops up, write your question (such as name, address or phone number).
There are two kinds of QR Files. The QR template and the QR Form file. The template is for designing and the form is for publishing. We need to save both of them. First, click on the save button and give the template a descriptive name. Next, hit the make button and save the form a place where you keep your published material. View the example. | Download the template. (Right click and "save target as") Note! on the internet it is good practice to have filenames in lower-case without spaces. If you want to design complex forms, Jacob Nielsen has a very informative site describing how to optimize web sites and e-mails. It is true that it covers a different purpose, but the techniques are still valid for forms as well. How to design e-mail and web sites.
